A lot of fetishists have one main thing that "drives them". However, you can also acquire "secondary fetishes" through exposure.
For example, the first times I saw a picture of a latex nun, I was like "that's kinda... odd". But a part of my brain was going "dude, that's a hot woman in latex, that's HOT!!!". I can't say that I have a fetish for nuns, but I do appreciate those images now. However, there are other things that are associated with latex that I definitely have developed a fetish for. High heel boots for example (on a woman, not wearing them).
This is one reason that for me, I have to be careful in a way - I don't want to develop a fetish for something that's wrong or dangerous. I think my brain was (and still is) vulnerable to fetishism, and that's probably biological (why are so many geeks kinky? because kink and geekhood have the same root: obsession). I spoke to him a lot - he's a very decent person. I'm not in his shoes, but while I agree with you it might not be unhealthy per say, it's a very heavy burden on this shoulders. First it's very, very misunderstood. In Montreal (ah, Montreal, how I love thee) a "club" for people with this fetish was open, and the general population reacted with outrage - they couldn't distinguish this kink from pedophilia, even though it's not the same thing *at all* (Montreal still has a way to go).
Because of this confusion, he could get fired from his job as he sometimes has contact with kids. It's also hard for him to get his partner to participate, as it's so... strange and queasy. He comes to the local club from time to time, (and now I'm speculating) not because he gets to "play" or has any hope of finding someone into it, but rather because he needs to go somewhere where he will be accepted.
so I agree with you on the unhealthy aspect - it just seems extremely difficult to live with. And yet, he's happier now that he has accepted who he is than when he tried to fight it.

Most psychological. I won't get into it much further than that, suffice to say, I have nightmares frequently enough that... erm... I end up using the diapers when I sleep (very infrequently- but it's happened).

OK, this makes more sense than it being a sexual thing to me. I can imagine feeling very small and vulnerable and wanting to retreat to a safe childhood, especially when you have not had one.

I'll admit, I was highly reluctant to share it, considering it hits many people's 'squick' factor, but I thinks it's something that people don't really understand, where it's easy to get the wrong idea what it's about. In a day and age where people scream loudly "think of the children!", any aspect of life which has traditionally been the province of children is considered horribly wrong for adults to intrude on. For a long time (even when I was a kid), the idea of adults playing with toys- dolls, action figures, etc. or reading comics or the like was considered weird for adults to do. Adults don't read comics! Adults don't watch cartoons! Adults don't play video games! Except... these days, it's pretty common. Our tastes have grown more mature as we get older, but we still try to retain a little of what we remember fondly from our childhood.
I'll admit, I don't expect everyone to grok what I do, but that's okay- it's a private thing, something I don't like showing to the rest of the world (although there are infantilists that are into humiliation and public displays, something I don't get, myself, but there are different types of infantilists).
I don't think people should be ashamed of who they are and what they do with their life, so long as it harms no one.

Edena, take it from me- most times sex isn't what it's cracked up to be. Many people seem eager to lose their virginity and experience it for the first time. But those experiences rarely are very fulfilling. When you find the right person, it can be mind-blowing, so waiting for that person to come along isn't so bad- it can be well worth the wait.
